I want to make a point that Vivi did try to get medbay and mining to help find the HoS's body and that was not at all reflected in either the original note or the amended note.

Nah, this is a valid note. You can't really prove intent in cases like this. Was it an accident? Maybe, but we can't for sure prove that it was, so we should probably keep record in case Vivi decides to shoot someone else with a syringe by accident.

Either we don't note for it and another "accident" happens, at which point we're at square one again, or we note for it and (if it really was an accident) it never happens again, so the note is just there for reference purposes.

I feel like I've been saying this a lot, but for minor instances of possible grief, a note by itself isn't a huge deal. If Vivi doesn't intend on shooting people with random syringes again, then it's not a problem. Nobody's going to think Vivi's a shitter for making a single mistake.

Notes are warnings for future admins, not an immediate punishment.

Every situation is case by case. I'm gonna continue to point out dents in skulls about this until people stop going note == ban. (challenge: impossible, not gonna happen)

note ≠ ban
however,
LOTS of notes of the same action repeated over and over again = ban

For recordkeeping purposes the note should absolutely mention whether he went to any effort to rescue the person he accidentally pwned, since that's extremely revelant in potential further cases.

its a better name only because its slightly more descriptive of what happened

you want to throw a whole bunch of your own personal pizzazz into it as well. thats where most of the points come from. sprinkle that shit with some glitter.
