make surgery a skill like mining gaming cleaning and the rest(the harder the surgery the more points rewarded towards leveling up)
levels up when you complete the surgery and seal(depending on contribution?)

This was a thing before, it was removed because people got mad about surgery taking too long if they weren't a doctor.
