Fuck object verbs, it's INTERFACE TIME.
One thing I've not seen (and have checked hotkeys-help for, thank you) is a run/walk toggle, as well as a jump/slash toggle to make spamming easier for allium hunters.

I was hoping at some point to make holding down shift make you walk and then remove the run/walk toggle button outright. Does that sound good?

Alt+clickings for (trying to think all the times I use right-click and get 3 second lag):
- Lock a closed locker even with an empty hand.
- Adjust breath mask, no need for the button.
- Flip cap.
- Toggle taserseclite light (no need for the uppercorner button).
- Alt + click (or maybe ctrl + click?) a tile with open airlock to close it - no more pixel-accurate airlock closing.
- Rotating a pipe.
- Rotating a window pane.
- Rotating a chair.
- Change Mining Satchel's gathering method (and other similar satchels if there are any).
- Toggle PDA light.
- Remove accessory on jumpsuit (medal or armband) and weapon (seclite).
- Cargo's Destination Tagger and detective's Forensics Scanner use the exact same sprite, which can be confusing.
- A pushed down breath mask could automatically jump back on your face when you enable internals, making it more of a aesthetic thing rather than a yet-another-stupid-death-thing. I always forget to push it back up before going spacewalking.

i can do everything you can with hotkey mode by using the arrows and the insert through the page down buttons, and i can use ctrl+whatever for anything else like intent selection, and at this point i couldnt switch to hotkey mode if i wanted to since the way i play is so burned in
however i would be down for a run/walk hotkey as long as the toggle stays in (wouldn't hurt to make the UI button for the toggle a bit smaller, it's kind of big for something that gets fairly little use)

I guess that's true. Arrow keys do have most of the same controls, even if a bit unconventional compared to most popular games these days. I still think hotkey mode should be the default, but a user's choice should also be saved. Actually ideally it'd all be rebindable but let's not get ahead of ourselves just yet. And in case that's all off topic, sorry.
Please remember that when defining hotkeys, it is an option to have different items do different stuff with the same hotkey. So hitting r with a gun reloads it while with a spray bottle it looks for something in front of you to refill with. Or whatever.
