Well it seems kind of silly to disallow carrying all items into a vent. I think a certain size of items should be allowed.

Most of these basically call for reverting things.
The reasons they were changed in the first place because it was recognized that they had flaws. I would suggest thinking in terms of forward progress instead of backwards if you think things need more changes.
I like the last idea though.

I think the armory window that leads to the equipment room should be replaced with a bolted airlock with officer level access and the only way to unbolt it is via a warden level access button inside the armory.

Right now science both serves as RnD and manufacturing. They can make the prototype AND mass produce it. If you could split those two functions up and give the latter to another department, (engineering?) perhaps the added inter-department dependency would slow things down a bit.

Pretty much agree with everything Anon said.
I just roll my eyes every time we get an AI that starts using ABORTIONS in every single vox announcement. It's not funny and frankly a bit annoying.

There's no reception desk, there's a lot of dead end hallways, there aren't any main hallway windows, the security post will most likely become more of an obstacle than anything, and the only thing open about it is the entrance. The interior is kinda labyrinthine.
